1. 护照页扫描件（务必清晰）

Clear scanned copy of International passport data page

2. 6个月以内的外国人体格检查记录

Physical examination record for foreigner within 6 months（附1/Attachment No.1）

3. 学历证明（一般为最高学历证明，即高中或者大学毕业证扫描件）

Scanned copy of Highest Degree Certificate (High school diploma or university degree certificate)

4. 最高学历成绩单

Academic transcripts of Highest Degree

5. 来华留学本科入学学业水平测试成绩单（仅申请本科课程需要提供）

The transcript of China Scholastic Competency Assessment（For undergraduate programs)

6. 6个月以内的无犯罪证明

Certificate of non-criminal record within 6 months

7. 当地银行开具的学生本人存款证明（能够承担在学期间学费、生活费、往返国际旅费的经济证明，每年至少一万美元）

The student’s personal bank statement issued by the bank in his/her country of residence (should be sufficient to fund tuition, accommodation and living expenses in China as well as travel expenses to and from China-----at least 10,000USD for a year )

8. 未满18周岁的未成年人需提供监护人委托书，并到中国大使馆公证

Applicants less than 18 years must provide a guarantee letter issued by a guardian in China and notarized by the Chinese embassy
